Support coordination helps to implement all possible supports in a participant’s plan, including: informal, mainstream, community and funded supports. Once a plan has been approved, participants work with support coordinators to determine how their funds will be spent and to connect with providers.

Support Coordinators work creatively and resourcefully with participants on how they utilise their support budgets to achieve their goals.

This is likely to include supporting the participant to:

  • Assess a number of mainstream, community, informal and provider options.
  • Choose preferred options or providers
  • Negotiate services to be provided and their prices, develop service agreements and create service bookings with preferred providers
  • Negotiate services and prices as part of any quotable support
  • Arrange any assessments required to determine the nature and type of funding required (eg. assessment to determine the type of complex home modifications required)
